I live in Indiana as well and my oldest son's father is not listed on his birth certificate. You need to have paternity established for your daughter. By signing the papers it will not relieve him of his responsibility. My son's father lived in KY and now in TN and although it took several years the state finally caught up with him. I am now married and have two other sons by my husband and believe me, in order to have my oldest son's dad relieved from his responsibility of being a dad it is going to take much more than a piece of paper!
